Facility Use Agreement – Terms and Conditions

Facility Use Application - Part 2

This form is to be completed after Part 1 of the application has been approved. Part 2 must be completed prior to facility use.
Changes to the agreement that can be negotiated are as follows: 1) Consumption of food or beverages - note that the consumption of alcoholic beverages is non-negotiable). 2) The use of nails, tape, tacks, signs, posters or other objects to hang or be placed on a Church surface must be identified and approved 3) The use of on-site storage space 4) Delivery of materials will not be accepted unless prior arrangements have been made. 5) That the renter can only be in the building at the times specified in the agreement, unless prior arrangements are made. Please indicate below which changes (as indicated above) you wish to negotiate, if any:

This signature section will be completed at orientation. Note that if the renter/user is not an authorized signing officer for the organization that is renting, arrangements will need to be made to obtain signatures from the authorized signing officer.
